WAIVER & RELEASE OF LIABILITY
For Participation at Academix Studio Inc. dba NEO Martial Arts
This agreement is entered into by the Participant (or their legal guardian, if under 18) and Academix Studio Inc., operating as NEO Martial Arts, including its directors, officers, employees, instructors, agents, volunteers, successors, and assigns (collectively, “the Academy”).

Acknowledgement of Risk
I understand and acknowledge that participation in martial arts, including Brazilian Jiu-Jitsu, involves inherent risks of physical injury, illness, or death. These risks include, but are not limited to: collision with persons or objects, falls, strains, sprains, broken bones, and other serious injuries. I voluntarily assume all risks associated with my participation in activities conducted by or affiliated with Academix Studio Inc. dba NEO Martial Arts.

Release & Waiver of Liability
In consideration of being allowed to participate in any way in programs, events, or classes offered by Academix Studio Inc. dba NEO Martial Arts, I hereby waive, release, and forever discharge the Academy from any and all claims, demands, liabilities, costs, or causes of action arising out of or in connection with any personal injury, property damage, or loss that may be sustained by me while participating, whether caused by the negligence of the Academy or otherwise.

Fitness to Participate
I certify that I am in good physical condition and have no known medical or other condition that would prevent me from safely participating in martial arts training. I understand that I am solely responsible for consulting a physician before beginning any training with Academix Studio Inc. dba NEO Martial Arts.

Rules & Conduct
I agree to abide by all rules, instructions, and safety guidelines established by the Academy and its instructors. I understand that failure to comply may result in disciplinary action or dismissal from the program.

Photo, Video & Media Consent
I understand that the Academy may capture photographs, video, and audio recordings during classes, events, and activities.
I hereby grant Academix Studio Inc. dba NEO Martial Arts the irrevocable right and permission to use my (or my child’s) image, likeness, voice, and performance in any promotional materials, advertisements, social media posts, publications, and other media for commercial or educational purposes, without compensation or further consent.

Indemnification
I agree to indemnify and hold harmless Academix Studio Inc. dba NEO Martial Arts from any and all claims, actions, liabilities, and expenses (including legal fees) arising from my participation, including those arising from my own negligence or misconduct.

Limitation of Waiver
This waiver does not release the Academy from liability for gross negligence, intentional misconduct, or situations where liability cannot legally be waived under Ontario law.

Severability
If any portion of this agreement is held to be invalid or unenforceable, all other provisions shall remain in full force and effect.

Governing Law
This agreement shall be governed by the laws of the Province of Ontario and the applicable laws of Canada. Any disputes shall be resolved exclusively in the courts of Ontario.

Acknowledgement & Understanding
I acknowledge that I have carefully read this waiver and fully understand its contents. I am aware that by signing this agreement, I am waiving certain legal rights, including the right to sue. I confirm that I am signing this agreement voluntarily and without coercion.
